Breast Reduction

Breast reduction surgery (reduction mammoplasty) and breast lift surgery (mastopexy) at Dr. Caspari's clinic in Munich – your specialist for breast reshaping surgery

Breast Reduction and Breast Lift

Large breasts can cause significant back pain as well as psychological burdens. Many women suffer from chronic neck and shoulder tension, skin irritation beneath the breast fold, and restricted physical activity. The causes are diverse – genetics, hormonal changes, pregnancy, or weight fluctuations can all lead to disproportionately large or sagging breasts. Breast reduction surgery, also known as reduction mammoplasty, provides lasting relief.

Dr. med. Peter Caspari, a specialist in Plastic and Aesthetic Surgery, offers individual surgical solutions – from breast reduction to breast lift surgery (mastopexy). With his specialization in breast surgery and thousands of successful operations, he masters a wide range of the most modern techniques. Whether you need a breast lift without implants or a combined approach, Dr. Caspari will find the optimal solution for your individual needs.

Breast Lift (Mastopexy) – Breast Lift Surgery in Munich

A breast lift, also known as mastopexy, is designed for women whose breasts have lost firmness and shape. Many women seek a breast lift after pregnancy and breastfeeding to restore their original breast contour. Depending on your situation, a breast lift without implants or a breast lift with implants may be the right choice.

During breast lift surgery, excess skin is removed and breast tissue is lifted to restore a youthful, natural shape. Breast lift scars are carefully planned to be as inconspicuous as possible – typically around the areola and in the breast fold. With modern suturing techniques and consistent scar care, breast lift scars fade significantly over time.

Breast Reduction Surgical Techniques – I-Incision and T-Incision Compared

Choosing the right surgical technique is crucial for an optimal breast reduction result. Dr. Caspari masters several proven methods and selects the technique individually based on breast size, skin condition, and desired outcome.

I-Incision / Lejour Technique (Vertical Breast Reduction)

The Lejour technique, also known as the I-incision or vertical breast reduction, is a scar-sparing method that results in only a scar around the areola and a vertical scar down to the breast fold. This technique is distinguished by its high safety regarding blood supply and wound healing. An additional advantage: no implants are required, as the remaining breast tissue is optimally shaped. The I-incision technique is particularly well suited for moderate breast reductions and breast lifts.

T-Incision (Bostwick Technique)

The T-incision, also known as the anchor or inverted-T technique, was significantly influenced by Dr. John Bostwick III. This method is the technique of choice particularly for large breast reductions and pronounced gigantomastia. In addition to the scar around the areola and the vertical scar, an additional scar is placed in the breast fold. The T-incision allows for the removal of larger tissue volumes and provides the best conditions for a shapely, lasting result in extensive reductions.

When Is Breast Reduction Surgery Recommended?

We generally recommend breast reduction from a cup size D onwards and in cases of pronounced ptosis (sagging breasts). From this size, medical reasons such as back pain, neck tension, or skin irritation frequently support the case for surgery. The assessment of breast volume is always individual, as tissue consistency can vary considerably depending on the ratio of glandular to fatty tissue.

Aesthetic improvement may also be desired for smaller breasts – for example, a breast lift combined with augmentation to optimize shape and volume. Whether the indication is medical or aesthetic: Dr. Caspari will discuss the best individual solution with you in a personal consultation.

Safety and Risk Minimization in Breast Reduction

As with any surgical procedure, breast reduction or breast lift surgery carries general risks such as wound healing problems, temporary changes in sensation, asymmetry, or post-operative bleeding. Dr. Caspari minimizes these risks through the selection of the safest surgical technique for each case, careful patient selection, and realistic goal-setting – for example, deliberately not reducing too much in order to preserve blood supply and natural shape.

For breast lifts, Dr. Caspari frequently uses GalaFLEX – a temporary internal bra made of absorbable material. This innovative support scaffold stabilizes the breast tissue in the first weeks and months after surgery and then dissolves completely, leaving no foreign body behind. Combined with implant augmentation when needed, this approach achieves excellent and long-lasting stable results even in lift procedures.

Breast Reduction Cost and Insurance Coverage

The breast reduction cost varies depending on the scope of the procedure, the chosen surgical technique, and whether a combined breast lift is performed. In Germany, breast reduction has been removed from the hospital catalog – even for large reductions. The previously common 500-gram-per-side rule no longer applies. Complaints such as back pain, tension, or spinal problems are generally not recognized by statutory health insurers as sufficient indication. The process of potential cost coverage usually takes many months and in most cases ends in rejection. Dr. Caspari will advise you transparently on breast reduction costs and breast lift costs during your personal initial consultation.

Frequently Asked Questions

It is important that you come to our practice 14 days before your procedure to have an ECG and a laboratory examination performed. In advance, we need an ultrasound finding of your breast. Please note that you should refrain from blood-thinning medications such as aspirin about 4 weeks before the planned surgery date. Alcohol and nicotine consumption should also be reduced during this time to support your health. Don't forget to obtain the prescribed medications in advance.
The procedure is performed under general anesthesia to ensure that you are completely relaxed and pain-free during the entire procedure. This allows the surgeon to achieve the best results while you are in a comfortable state.
The duration of the procedure varies depending on the surgical technique used, usually 2-3 hours.
Afterwards, you will spend one night in our Plastic & Aesthetic Surgery Clinic, where you will be excellently cared for by our expert team.
After a breast lift or breast reduction, it is very important to wear the compression bra for 6 weeks. This supports the healing process and ensures the best possible shape of your breasts. During this time, you should take it easy physically to ensure optimal recovery. The suture material used will be removed 14 days after the procedure, which is another step in your healing process.
The first results of a breast reduction or breast lift are visible immediately after the procedure. Once the swelling and any bruising have subsided, the final result usually shows after about 3-6 months.
After a breast reduction or breast lift, patients are usually immediately socially acceptable, but should consistently take it easy in the first 14 days.
Breast reduction scars and breast lift scars depend on the technique used. Common patterns include a scar around the areola (periareolar), an additional vertical scar, or a T-shaped scar for larger reductions. Dr. Caspari places great emphasis on aesthetic incision planning. With proper scar care, the scars fade significantly over time.

Complaints such as back pain, neck tension, or spinal problems are generally not recognized by statutory health insurers as sufficient indication for cost coverage. Yes, a breast lift with implants is a frequently chosen option when volume restoration is desired in addition to the lift. This combination is particularly suitable for women after pregnancy and breastfeeding who wish to achieve both a breast lift and volume gain. Whether a breast lift without implants or with implants is right for you will be individually decided during your personal consultation.
Breastfeeding after breast reduction is generally fully possible with the techniques we use. Dr. Caspari is internationally renowned for his tissue-preserving surgical techniques and has presented his methods and results at medical conferences worldwide. We deliberately avoid techniques that could compromise breastfeeding ability. For patients planning to have children, we recommend ideally scheduling the breast reduction after completing family planning, as the breast can still change due to hormonal shifts during pregnancy.
After complete healing, physical activity is fully possible again without restrictions. The normal healing phase lasts approximately 6 to 12 weeks. From about 6 weeks after surgery, most patients can begin light exercise. Sports with strong vertical movements such as jogging or horseback riding can stress the breast shape and should be supported with a well-fitting sports bra to maintain optimal long-term results.
The desired target size is discussed together with you during your personal consultation. Using example images, we can give you a realistic impression of the achievable result. We deliberately do not use computer simulations, as these often create unrealistic expectations and tend to unsettle patients rather than support them. What matters are your individual wishes, anatomical conditions, and realistic goal-setting, which Dr. Caspari can optimally assess with his many years of experience.
With stable body weight, the results of a breast reduction are very long-lasting. While natural skin aging continues, the surgical result remains permanent. Significant weight fluctuations or pregnancy can alter the breast shape. We therefore recommend not planning a pregnancy within the first 12 months after a breast lift, in order not to overly stress the skin. Should a correction be desired at a later date, a repeat procedure is generally possible.
