When Is a Breast Implant Exchange Necessary?


A breast implant exchange becomes necessary or advisable in various situations. The most common reasons that bring patients to our clinic are capsular fibrosis in the various Baker grades (I–IV), breast hardening, and asymmetries caused by scar contracture or tissue changes. If the implant is older than ten years, we also recommend a specialist examination and, if necessary, an exchange.


Further indications for an implant exchange include breast deformation, implant displacement (dislocation), implant rupture, and changes in the surrounding breast tissue. The desire for a different implant shape or size is also a common reason. In every case, we clarify in a personal consultation whether a breast implant exchange is the right step for you.

Capsular Fibrosis – Causes, Diagnosis and Treatment


Capsular fibrosis is a hardening of the connective tissue capsule that naturally forms around every breast implant. Over time, this capsule can contract, leading to hardening, pain, and visible changes in breast shape. Capsular fibrosis is classified into Baker grades I through IV – from a barely noticeable capsule thickening (Baker I) to a distinctly hardened and painful breast (Baker IV). Diagnosis is made through clinical examination combined with imaging procedures such as ultrasound or MRI mammography.


At our clinic, we perform a complete en bloc capsulectomy for capsular fibrosis. The hardened capsule is removed together with the old implant as a whole – a procedure that requires the highest surgical precision. The risk of recurrent capsular fibrosis can be minimized by performing a thorough capsulectomy and using exclusively high-quality implants from reputable manufacturers, for which neither an increased risk of lymphoma (BIA-ALCL) nor implant rupture is known.

Implant Removal Without Replacement (Explantation)


Not every patient wishes to have new implants placed after the removal of their breast implants. Implant removal without replacement – also known as explantation – is of course possible at our clinic. Since volume is lost after the removal of implants, we recommend an accompanying breast lift (mastopexy) in most cases to achieve an aesthetically pleasing result.


The majority of our patients choose an implant exchange with new, high-quality implants. Some patients – particularly those who prefer a somewhat smaller but firm and natural breast – consciously opt for removal only in combination with a lift. Dr. Caspari will advise you individually on the options and the expected outcome.

Combining Implant Exchange with Breast Lift


In many cases, it is advisable to combine the breast implant exchange with a breast lift (mastopexy). This is particularly recommended when a smaller implant is chosen or when the skin has already been significantly stretched by the long presence of the old implant. The combination of exchange and lift enables an optimal result: a firm, full and naturally shaped breast.


The additional lift procedure extends the operating time by approximately 30 minutes. The recovery time remains comparable to a pure implant exchange. In a personal consultation, we will discuss with you whether a combination of both procedures is recommended for your individual result.

Surgical Technique and Procedure for Implant Exchange

For a breast implant exchange, we use – whenever possible – the existing implantation scar from the first operation as the access route. This has the advantage that no additional scar is created. The existing connective tissue capsule is completely removed as part of a capsulectomy to create a clean foundation for the new implant.


It is often a recommended measure to change the implant position – for example, from above the muscle (subglandular) to below the muscle (submuscular) – in order to prevent a recurrence of capsular fibrosis and ensure a long-term stable result. For the exchange, we exclusively use implants of the highest quality standards from reputable manufacturers such as Motiva or Polytech.

Before an elective surgical procedure, it is recommended to have a blood laboratory test and an ECG performed about 2 weeks in advance. BIA-ALCL (Breast Implant-Associated Anaplastic Large Cell Lymphoma) is a very rare cancer of the lymphatic tissue that has been linked to certain textured breast implants – particularly implants from the manufacturer Allergan. At our clinic, we exclusively use implants for which no increased BIA-ALCL risk is known. If you carry implants from an affected manufacturer, we will be happy to advise you on the next steps.
